Foot & Ankle FAQ

Unfortunately, due to confidentiality regulations, we are unable to give test results to patients over the phone.

Additionally, for the wellbeing and confidentiality of individuals, clinical staff is not allowed to give medical advice to non-patients or new patients prior to their first visit.

We encourage you to contact your agent or case manager to ensure coverage of this visit.

No, however you do need to have your insurance information handy when scheduling an appointment.
Before your initial visit, please plan on arriving 20 to 30 minutes before your scheduled time in order to complete the registration process.
Please bring your current insurance information and any recent test results, x-ray/CT/MRI discs, and a complete list of your active medications.
In order to schedule an appointment please call our main office at (616) 459-7101. We are located at 1111 Leffingwell Grand Rapids MI, 49525 and are open 8am to 5pm, Monday through Friday.
